Employee amputates fingers while cleaning ungarded auger
On October 31 2003, Employee #1 was cleaning an auger. The employee removed the cover, exposing an unguarded auger. Employee was not informed that the guard had been removed. When he placed his hand into the opening, the auger amputated Employee #1's right middle and ring finger.
