Employee Run over and Killed by Truck

Employee #1 was holding the lease gate open to allow the crude hauling tractor trailer rig to pass through leaving the lease site. This gate was fitted with 3 metal spikes each about 24 in. long at approximately 30 in. above ground level. The gate is used to prevent drivers from pushing the gate open with the front bumper of their truck. All indications are that Employee #1 was closing the gate when one of the spikes came into contact with one of the passenger's side rear dual wheels on the trailer. This knocked Employee #1 forward and into the oncoming tires crushing him to death.
