Employee's Finger Is Crushed by Forklift Mast

At approximately 11:00 a.m. on August 2, 2006, Employee #1 was operating a Daewoo G25S forklift and was attempting to empty a trash bin into a dumpster. The bin would not tip to dump the load. Employee #1 got out of the driver's seat, climbed onto the right front fender, grabbed the mast with his left hand, and attempted to loosen the bin with his right hand. Three fingers of his left hand were crushed by the mast. The employee was taken to the hospital. During a follow up visit on August 2, 2006, it was determined that Employee #1's left index finger would need to be amputated at the bottom joint.
