Investigation Summary
At approximately 12:00 p.m. on October 26, 2005, Employee #1, a new employee, was operating a Komatsu 25 forklift, Model Number FE25T-1, Serial Number 505127A, for which he was not trained or certified, to move it from blocking the exit path of a vendor's truck. As he was driving the forklift, he collided with the left rear corner of the vendor's truck flatbed. His hand, which was outside the forklift's line of travel, became caught between the left rear upright of the forklift cab and the left rear corner of the flatbed trailer, resulting in a laceration of his left hand with a crush fracture of the left small finger knuckle, and damage to the nerves of the left hand. Employee #1 was taken first to Bayside Medical Center, then to Century City Doctors' Hospital, where he was hospitalized for three days for the crush injury and nerve damage.
