Employee is injured after falling from I-beam

On June 3, 2004, an employee and a coworker were installing a GLULAM, a laminated beam, between two cross I-Beams. This GLULAM was 15-ft-long, 15-in.-high and 3.125-in.-wide. The top of each I-Beam was 10-in. wide, on which the employees and the coworker were standing. The GLULAM beam was positioned in place with a Gradall forklift and the employee and the coworker were inserting ends of this beam into the hanger pockets previously installed on the I-Beams. While trying to push the end of the GLULAM into the pocket, the employee lost the balance and fell 13 ft 8 in. down to the concrete floor. The employee suffered injuries to his head, neck and right eye, when he fell on his head. He had eye surgery to repair the damage and was hospitalized for two days.
