Employee injured after falling 14 feet

On May 10, 2004, an employee was placing decking material. An engineered catenary system was in place for employees to tie off. The employee was wearing his harness and did not want to use his lanyard to connect to the catenary line because it limited his movement. The supervisor told the employee to set the decking panels in place and knew that the employee was not tied off. The 3-ft-wide by 15-ft-long fiberglass decking panels were just set on the beams. The leading edges of the panels were not tied in to the beams. The employee turned on one of the leading edges of a panel setting on a beam. The panel moved and the employee fell 14-ft down, landing on the concrete floor. The employee sustained a fractured right hip that required pinning, fractures of lower vertebrae and a fractured skull. He was hospitalized over 24 hours.
