Investigation Summary

Investigation Nr: 201143096
Event: 01/25/2002
Employee Partially Amputates Thumb in a Machine

On January 25, 2002, Employee #1, a temporary employee, was assigned to check automobile starters. He found one that had not had its core removed. A coworker told him to set it aside, but instead, Employee #1 went to a small ENERPAC bearing press that was used to remove cores and removed the core himself. He had not been trained in the use of the press. He was holding the starter as the slow moving press came down. When it contacted the starter, the part twisted, causing his left thumb and index finger to be squeezed between the part and the press, partially amputating his left thumb and fracturing his left index finger. Employee #1 was hospitalized for his injuries.
