Employee killed in fall from ladder

On December 31, 2001, Employee #1 was checking the company inventory for parts located on a shelf in the show room. He was working from a 6-foot Werner fiberglass step ladder (five rungs and a top step) that was placed near the shelf. He was reading part numbers from boxes and was recording them on a piece of paper. There was no witness to the fall, but another employee, the co-owner, found Employee #1 laying on the floor next to the ladder, which was tipped on its right side. His feet and legs were tangled in the second rung of the ladder. Employee #1 struck his head on the floor, which was concrete covered with carpeting and a rug, and was killed.
