Investigation Summary

Investigation Nr: 201201548
Event: 10/12/2001
THE EMPLOYEE HAD HIS LEFT HAND AMPUTATED.

The millwright (victim) was located under the Chargers operator's deck, attempting to find the cause of the malfunction of the "Vees". The head electrician was located on the upper deck. The electrician was attempting to move the Vees up or down by activating the solenoid by hand. After the fourth or fifth time of trying to move the Vees, the electrician heard a loud scream from under the deck where the victim was located. The victim thinks he may of stuck or layed his left hand into an inspection port, and as the Vees were activated they came down and amputated his hand by a shearing motion.
