Mechanic injured in fall

At approximately 2:30 p.m. on July 10, 2001, Employee #1, a mechanic, was working on a swing stage scaffold with a foreman, hauling counterweights from the ground to the roof of a 16-story condominium. Near the top of the building, the work platform board slipped out of the scaffold's stirrups, and fell to the ground, leaving Employee #1 and the foreman dangling by their lifelines. As a result of the fall while wearing his harness, Employee #1 sustained a sprain and missed two days of work. Investigation of the accident found that the bolts holding the work platform portion to the scaffold were not inspected for tightness by a competent person before use.
