Employee is killed after falling from ladder

At approximately 11:30 a.m. on December 7, 2001, Employee #1, a storage facility worker, was preparing to change a light sensor near the top of the roof. The employee extended a 25-ft fiberglass extension ladder against the storage building. When his supervisor went outside she noticed that he had fallen from the ladder and had been killed.
