Employee killed in trench cave in

Employee #1 and a co worker were working in a trench, 13 ft deep by 11 ft wide and 17 ft deep, installing a sanitary sewer manhole. The trench did not have any cave in protection and the North side wall collapsed. Employee #1 received fractured ribs, collapsed lungs and heart and liver damages and died.
