Employee Injured In Fall From Ladder

Employee #1 was on an extension ladder removing an exhaust duct that was made of metal pipe and was running through the roof of the generator building. The ladder was separated into two sections and only one half was used. This part did not have slip resistant feet and was not secured. It was placed on concrete. The ladder slipped and Employee #1 fell approximately 8 ft to 9 ft to the concrete. Employee #1 was hospitalized.
