Employee Injured When Tractor Overturns

At approximately 1:00 p.m. on April 28, 2000, Employee #1 was using a Case Tractor, Model Number 480 LL, Serial Number 17401679, and Identification Number 17001316 to build an oblong dirt mound that measured approximately 60-ft-longby-38-ft-wide by 4-ft-high. The mound was constructed to hide the maintenance yard work operations from the Club House. Employee #1 approach the dirt mound with an extended bucket of dirt and after pulling up the side of the mound and releasing the load, the center of balance of the tractor shifted causing the tractor to overturn to its left side. Employee #1 was partially confined under the roll cage of the tractor and suffered multiple head injuries. Employee #1 was hospitalized.
