Investigation Summary
On November 16, 1999, Employee #1 was applying a foam coating to a flat industrial roof. When another employee asked him for the power sweeper on the roof, he then made a hoist for it from parts around his vicinity and began lowering the sweeper to the employee on the ground. While controlling its decent, he lost his balance due to its weight and fell approximately 22 ft to the ground. He was not wearing any fall protection gear and the hoist was unsecured. The impact of the fall caused him to receive traumatic brain injures resulting in the loss of consciousness. He was transported to the hospital for emergency treatment, but later succumbed to his injures and died because of them.
