Employee injured when mica bin and stand overturns

Employee #1 was working adjacent to an over 700 lb mica metering bin mounted atop a 24 in. by 20 in. by 75 in. tall equipment stand. The stand had four swivel wheels, one on each corner, and approximately 21 in. apart. The stand and mica metering bin accidentally tipped over and fell on Employee #1. He sustained crushing injuries, for which he was hospitalized. The stand was very top heavy.
