Investigation Summary

Investigation Nr: 170827034
Event: 02/16/1999
Employee injured in fall after slipping on loose roof panel

Employee #1, age 19, was strapping up stacks of old roof panels for a crane to lift off the roof. A stack that had already been raised was being boomed over the roof when Employee #1 noticed that it was going to catch on the corner. He went to swing the bundle out when his lanyard became caught on another stack of panels still on the roof. Employee #1 unhooked his lanyard from his harness and was attempting to snap it loose from the stack when he slipped on a loose panel and fell approximately 29 ft 3 in. to the ground. Employee #1 was transported by ambulance to Washington County Hospital, where he was treated for a fractured left wrist, left pelvis, and left ankle.
