Investigation Summary
On October 30, 2000, Employee #1 was lifting a motor shaft with a crane attached to a Jergens center - pull swivel. As the shaft was lifted the bolt holding the swivel to the motor shaft pulled through the swivel ring causing the motor shaft to fall smashing the Employee #1's left ring finger and little finger between the metal cart which the shaft was being lifted from and the falling shaft. No fractures resulted from the accident. The fingers were bruised and separation of the nail bed occurred on the ring finger. Employee #1 was injured and not hospitalized.
