Event: 08/20/1998
Employee killed in a fall from a moving vehicle

At approximately 12:30 p.m. on August 20, 1998, Employee #1 and several workers were riding on the back of a flatbed truck. They were going home for lunch. The workers were riding standing up and were holding onto the backboard located right behind the truck's cab while Employee #2 drove. Towards the rear area of the truck bed were six unsecured woodbins. A few of the workers were holding onto these woodbins to keep their balance. As the truck approached a curve on the roadway, Employee #2 slowed down to enter the curve. When the truck was almost out of the curve, Employee #1 slipped on the metal decking of the truck bed and slid off the truck. He landed on the roadway shoulder and into a ditch. The other workers got the attention of Employee #2 and asked him to stop. After the truck came to a stop, the workers jumped off the truck and ran to assist Employee #1. A passing motorist used his cellular phone to call 911. An ambulance arrived to the scene and provided Employee #1 with medical care and treatment. Employee #1 was transported to a hospital were he died from fatal injures to his head in the fall.
