Investigation Summary
Employee #1 was repairing a flat on a forty-six-inch tire. He put an air chuck on the tire valve and walked away to get a jack from the service truck. He took about five minutes to place the jack under the vehicle and had his head next to the tire. The tire exploded. The service truck compressor is capable of 150 pounds per square inch.
