Employee killed when crushed by shifting load of glass

At approximately 1:00 p.m. on February 22, 1999, Employee #1 was pitching 3 ft by 6 ft pieces of glass from the back of a Chevrolet Custom Deluxe flatbed truck. The glass panes were attached with a 9/16 in. rope to a metal A-frame on the interior of the truck. Three glazers receiving the glass were setting frames and installing glass about 50 ft away. They left Employee #1 working alone for 3 to 5 minutes. One of the coworkers then returned to the truck and found Employee #1 pinned underneath a 1,080 lb stack of glass. He died of crushing injuries.
