Employee injured in an industrial truck accident

At approximately 5:30 p.m. on August 5, 2003, an employee, a control specialist, who had four months experience working at this job. The employee was closing up the warehouse, a required assignment. He was not performing a normally assigned task, when the accident occurred. The industrial truck involved in the accident was a Crown stand up forklift Make Number 35 RRTT, Serial Number H17939 as per maintenance records. The employee suffered a crushed left heel, with multiple fractures and severe lacerations, requiring 2 debridement surgeries. He was hospitalized and treated for his injuries. He stated that the brakes did not seem to work. After the accident, the employee was transported to a nearby hospital by his operating manager. He stated that he had not checked the industrial truck prior to driving it. He stated that he had not received training nor had he been certified as an operator by his employer. There were no written procedures for duties on the forklift. This accident was reported to this office on May 21, 2004, nine months later.
