Employee run over by steerable dolly

Employee #1 talked with a coworker who was operating a steerable dolly attached to a semi-tractor trailer loaded with a 100-ft overhead crane. Employee #1 positioned himself between the operator's platform and the tires of the dolly, separated by approximately 2 ft. Just prior to Employee #1's arrival, the operator of the dolly instructed the driver of the truck to move ahead. The tires of the dolly trapped the foot of Employee #1 and pulled him to the ground and ran over his legs and lower half of his body. Employee #1 was taken to the hospital where he died.
