Investigation Summary
At approximately 6:50 a.m. on February 25, 1997, Employee #1 was sanitizing a roll mixer. She carried a bucket of soapy water weighing approximately 15 pounds to the machine and used it to sanitize the surrounding area. Coworkers saw Employee #1 moving slowly and told her to sit down. Employee #1 sat for a period, and had then gotten up to continue working when she collapsed. An ambulance was called to the site. Employee #1 was transported to the hospital, where she was pronounced dead.
