Investigation Summary
Employee #1 went up on the metal roof of a building to install a vent for heating. While on the roof Employee #1 encountered slick conditions and slid down the roof and off the eaves, falling 16 feet from the roof eave to the ground, fracturing both ankles. Employee #1 was hospitalized.
