Investigation Summary
On July 20, 2011, Employee #1 was working for his employer, a mold remediation contractor that was removing mold from the inside walls of a single family residence. Employee #1 was assigned the task of trimming a shear panel with a Milwaukee brand, model 6130-33, 4 1/2 inch grinder. The grinder had been modified by removing the grinding wheel and substituting a 3 3/8 inch by 24 inch carbide tipped saw blade. Employee #1 trimmed the panel, and made a cut of approximately six inches. Employee #1 turned off the grinder, and the grinder slipped out of his right hand as it was still freewheeling. Employee #1 caught the grinder with his left hand and the unguarded blade caught his middle finger. Employee #1 cut his middle finger and broke the distal phalanx. Employee #1 was transported to a medical clinic where the soft tissue was stitched and the bone was set.
