Investigation Summary
At approximately 12:00 p.m. on November 18, 2009, Employee #1, an installer with Tempo Air Conditioning and Heating Services, was on the second floor of a residential building being remodeled. He was tossing cardboard boxes containing fiberglass ducts down to the first floor level, where the lead installer was retrieving them and walking them to a storage room. As Employee #1 was about to toss one of the boxes down, he noticed the lead installer walking into the path of the box. Employee #1 attempted to restrain himself with his left hand against a horizontal 2 in. by 4 in. guardrail. When the guardrail failed, he fell approximately 9 ft to the first floor. Employee #1 was hospitalized at Cedars-Sinai Medical Center for examination and observation purposes only. The accident could have been prevented if the guardrails were properly installed.
