Employee Sufferes Crushing Injury And Amputation

At approximately 12:48 p.m. on November 28, 2000, Employee #1 was unloading a truck and placing the load on the loading dock located next to the ramp located on the southwest corner of the building. After unloading and placing the second pallet on the loading dock Employee #1 backed up and positioned the forklift to drive forward up the ramp. Employee #1 was almost all the way up the ramp when the forklift started to slip. Employee #1 hit the brakes, but the forklift slipped back due to the ramp being wet. Employee #1 lost control of the forklift and it slid over the edge of the ramp and tipped over. Employee #1 extended his hands to the ground in an attempt to break the fall, and the overhead guard of the forklift hit his left hand. Employee #1 suffered a crush injury which resulted in the amputation of three fingers. Employee #1 was hospitalized.
