Investigation Summary
Employee #1 was working at a demolition site tearing down an elevated railroad bridge structure. He was standing on a beam/girder that had been cut loose at the opposite end from where he was positioned. The beam, along with Employee #1, fell 30 ft to the ground. The beam landed on the employee, crushing his chest and killing him.
