Employee killed in fall from stepladder

Employee #1 was on an 8 ft stepladder, with a working height of 5 ft 9 in. He was changing fluorescent light bulbs and possibly repairing the light fixture when he fell from the stepladder onto the sidewalk. Employee #1 was killed. Coworkers later stated that that they were permitted to stand above the highest allowable step on these ladders to gain access to the light fixtures.
