Three employees injured when struck by falling storage tiers

One January 27, 1997, Employees #1 through #3 were setting up tiers to store medical records. Employee #1 was pushing microfilm boxes back onto the rear tier, which was located at the wall in the microfilm area, when the tiers began to sway and fall toward him. This caused the tiers in front of him to also fall. They struck Employees #2 and #3, who were securing several other tiers in front of the microfilm tiers. Employees #1 through #3 sustained injuries to their legs, arms, face, and other extremities. The tiers were not interlocked or secured.
