Investigation Summary
Employee #1 was replacing the wearplates on a shear. He welded a 9 in. wedge-shaped steel section to the side of the throat opening for the "clamp" head area. The steel section was to support the 9,000 lb clamp in the raised position so the deck wearplates could be installed beneath the clamp. Before he left this part of the work area, Employee #1 cut approximately 7 3/4 in. of the weld on the wedge, leaving 1 1/4 in. to support the entire clamp. He was removing his tools when the "clamp" head fell 18 in., landing on the newly installed deck. The force of its impact created a percussion shock that severely crushed Employee #1's left ankle. The employer did not have a lockout/tagout program, and he had neither trained his employees nor provided them with the proper hardware.
