Employee injured in fall from platform

Employee #1 had pumped up the right side of the pump jack scaffold when a coworker on the ground reported that the platform was hanging 1/2 in. over the jack bracket. Employee #1 walked to the left side of the platform to even out the brackets and to pull the platform so that each side had excess hanging over the bracket. The platform kicked out from underneath him, and he fell 15 ft onto the privacy fence. Employee #1 had not installed the pump jack scaffold himself and he assumed that the foreman had installed the system correctly. There were no guardrails, the poles were not braced, the platform was not fully dried and sanded, and the employees had not been trained in the set up or use of a pump jack scaffold.
