Employee's fingertip amputated in press

Employee #1, age19, was using a Minister #6 mechanical press, (company press #5), to stamp holes in hollow square metal bars. He was working alone, and he was using the foot pedal instead of the two-handed switch. A bar that was being stamped had not punched cleanly, and Employee #1 used a tool to try to get the punched piece from the stamped bar. He could not remove it with the tool, so he tried to release it by hand. While his hand was in the machine, the mechanical press cycled, amputating the tip of his left index finger. There was no guard on the die or on the press to prevent workers from putting their hands into the point of operation. At the time of the accident, Employee #1 was a little over an hour into his shift and had been working on the same press for the entire time.
