Employee killed in fall from header beam

On July 12, 2007, Employee #1, the on-site lead person, was removing and replacing termite-damaged wood around some large picture windows at a duplex rental property in Playa Del Rey, CA. He was in charge of replacing a window frame that was behind a 4 ft wide balcony. A 3 ft wide scaffold had been erected for support. Employee #1 was standing on a 4 by 4 header that was 6 ft 10 in. above the floor of the apartment and the balcony when he suddenly pitched outward across the balcony and fell 25 ft, landing on his head. He suffered severe head injuries and was removed from life support the following day. Employee #1 died of injuries sustained in the accident.
