Employee injured when gate derails

On November 15, 2001, Employee #1, a maintenance worker, was installing a new gate in the entrance of an apartment complex. He positioned the lower rail across the entrance, but failed to place a stop on the rail, which would have prevented its derailment. However, as the gate closed it derailed and tipped over, falling on him. The gate weighed several hundred pounds and caused multiple injuries. He was transported to the hospital for these injuries, which consisted of fractured ribs, a punctured lung, and lacerated arm. Because of the damage to his lung, he was hospitalized for six days to receive surgical and postoperative care.
