Investigation Summary
At 4:00 a.m. on August 15, 2007, Employee #1 started his shift and he was supervising and putting tools together for jobsites. Employee #1 was mechanical manager and he had worked for the employer for 3.5 years. A witness started his shift at 5 a.m. and he changed teeth on the auger till 6 a.m. Employee #1 came over to the witness and they loaded the auger on the truck. Employee #1 told the witness to remove the bolt that was holding the end bit to the auger. The witness cut off the bolt using a torch and hammered out the remaining piece. Employee #1 was operating a forklift to suspend the end bit and auger in the air. The end bit was chained to the fork of a forklift and Employee #1 was moving the fork in and out to separate the end bit from the auger. They were doing this for 10 to 15 minutes when the witness saw white steam coming out from the bottom of the forklift. Then the radiator cap located behind the driver's seat blew off and hot radiator fluid spewed out and splashed over Employee #1's back. Employee #1 jumped out of the forklift and the witness climbed up to the driver's seat to turn off the engine of the forklift. The witness offered to take Employee #1 to the clinic and he declined. Employee #1 went to the office to report the accident and he went home. Three to four hours after the accident, Employee #1 noticed that the injured area was starting to blister and his girlfriend took him to Fountain Valley Community Hospital from which he was transferred to UCI Burn Center. Employee #1 was diagnosed with second degree burn injury on his back and back of his head and he was hospitalized for 4 days. The forklift was manufactured by Yale Materials Handling and its model number and serial number were G51P-040-CFS and L155444 respectively. The dimension of auger was 14 in. in diameter by 67 ft long including the end bit, which was 2 ft long.
