Employee run over by forklift

On March 27, 1999, Employees were transporting a large case of glass, rigged to a carrier attached to the forks of a forklift. The case was approximately 6 feet square and suspended so that it was free to swivel. Employee #1 was walking alongside to steady the load, a coworker was in front of the truck and load, and another coworker was driving the forklift. During transport the case started to rotate and Employee #1 stepped in between the load and the forklift truck. As he did, the forklift truck tire caught his heel causing him to fall. It then ran over both his feet, breaking both of them. Employee #1 was hospitalized for the fractures.
