Investigation Summary

Investigation Nr: 201111481
Event: 11/12/1998
Employee injured when fell from a ladder

On November 12, 1998, a serious accident occurred at the construction site of the Commerce Center Water Tank project. Employee #1, a welder, was working inside a tank on a ladder while the crew was moving scaffold from the second "ring" of the new tank to the third ring. A crane was sitting outside the tank and was hoisting material inside the tank. As the crane was lifting some scaffold planks, the rigging became entangled with the ladder that Employee #1 was working from. The crane hooked the ladder and dislodged it, and Employee #1 fell. He sustained a jaw fracture and was taken to Henry Mayo Hospital for treatment. He was hospitalized.
