Investigation Summary
Employee #1 had just finished the last quality control inspection on the last cow through the lines as the company finished up production for the day and the crew prepared to go home. He was not paying attention as he went upstairs to his office. He was passing through the door to the kill room when he inadvertently walked into an overhead tripe tree meat hook rack that the clean-up crew had put on a separate rail next to the door for sanitizing. Employee #1 lost his right eye in the accident.
