Employee's fingers amputated in metal shear

Employee #1 returned from making deliveries and had begun his other duty of cleaning up after the welders at the plant. He was working in the shop area when a welder asked him to shear a few small pieces of metal. Employee #1 sheared all the parts but one, and then held the last part on both the top and the bottom so he would not have to go around the machine to pick it up. The stock metal clamp engaged, smashing two of his fingers against the bed of the shear. The tips of both fingers were amputated, and he was taken to the hospital for treatment. Employee #1 had not been trained to operate the metal shear.
