Investigation Summary
On February 23, 2004, Employee #1, a lineman-tree trimmer, was preparing to trim a 10-ft to 12-ft tree. Employee #1 was in the tree and the portion of the tree that he was attached to broke. Employee #1 fell to the ground. He fractured his back and was taken to a hospital.
