Employee's fingers amputated in table saw

At approximately 2:00 p.m. on June 12, 1999, Employee #1, of D & D Cabinet, was using a 10 in. diameter table saw to rip 3 in. long by 22 in. wide lumber strips into 2 in. wide strips. After making a cut, he used a push stick to guide the lumber through the saw. Employee #1 was reaching around the back of the saw to grab the piece of lumber when it caught the saw blade and was pulled back through. Employee #1's hand was also pulled into the saw blade, amputating two of his fingers. Coworkers called 911 and the fire department provided first aid before he was taken by ambulance to Sutter Roseville Medical Center in Roseville, CA. Employee #1 was then transferred to UC Davis Medical Center, where one of his fingers was reattached. The table saw was not equipped with an anti-kickback device or splinter at the back of the saw blade.
