Employee injured when sleeves caught by rotating shaft

On August 31, 1999, Employee #1 was standing on a work platform, cleaning debris from the prune dryer/shaker. She was reaching for some sticks when the right sleeve of her T-shirt became caught by the rotating shaft extension of the offset shaker shaft. Employee #1 suffered severe bruises to her right breast, resulting in hematoma and blood clots. She also fell during the accident, lacerating the back of her head. Employee #1 was a forklift driver for an operation that grows and dries prunes.
