Employee killed while moving industrial truck

On April 14, 1995, Employee #1, of Advance Transportation Company, was operating a powered industrial truck. As he attempted to exit a docked trailer, the tractor trailer pulled away from the dock. The powered industrial truck fell out of the rear of the trailer. Employee #1 received head injuries which resulted in his death. A citation as issued for serious violations of 4080-1011(a) and 4081.2154(1).
