Employee killed in fall through roof panel

At approximately 8:30 a.m. on May 26, 1999, Employee #1 was helping to install a standing seam roof when he walked out onto an unsecured roof panel and fell 25 ft 8 in. He was killed. No form of fall protection was being used by the workers installing the roof or by those staging materials on the completed portion of the deck. At the time of the accident, Employee #1 had worked for the company for approximately two weeks.
