Investigation Summary
At 11:55 a.m. on Dec 18, 2003, Employee #1 was working on notcher machine number 534, which produced pallets, as part of his regular duties. Although the machine was turned on, the motor did not start. Employee #1 attempted to make the motor run by pushing the drive belt with the fingers of his right hand. The belt started moving, catching and amputating Employee #1's right ring and middle fingers. Employee #1 was taken to Antelope Valley Hospital and then was transferred to Northridge Hospital later that night. Employee #1 was hospitalized for three days.
