Employee injured in fall into auger

Employee #1 was cleaning a Reitz mixer while it was running. He was standing on top of a 3-step ladder approximately 30 inches tall, when he fell partially into the auger of the mixer. Employee #1 suffered partial amputation of his left hand and a compound fracture of his upper left arm. Causal factors were the employee standing atop a 30-inch stepladder and the fact that the auger on the Reitz mixer was running and unguarded.
