Investigation Summary

On October 22, 2004, Employee #1, a teacher, was working at a church that operated a day care and school for ages 2 thru 12. He lived on campus and was attempting to repair a leaky roof on one of the modular classrooms. He accessed the roof by an extension ladder. He had placed some roofing material on the flat top and was walking toward the ladder when his foot hit a metal electrical conduit causing him to loose his balance. The conduit was less than 2 ft from the edge of the roof and when he lost his balance he fell off the roof 12.5 ft to the ground. He struck his head on the concrete below and received serious head injuries and was hospitalized. Employee #1 eventually died.
