Investigation Summary

A heavy shoe-sized box, neatly wrapped and automatically printed, was delivered by US mail to a retired association director, who had retired two years before. The postal carrier placed the package on top of the day's mail on a reception desk in the front office of the association. Various workers looked at the package but did not attempt to open it because it was addressed to the previous director, and it was wrapped in fabric tape, which is hard to open. Everyone left the reception area, except the current director. When he opened the package, it exploded, killing him instantly. No other workers were injured by the blast.
